Primary Care
Our patient-focused primary care is delivered by skilled, experienced professionals focused on your overall well-being, not just a single problem or illness. For adults and qualifying seniors, we are here to answer your health questions, provide preventive care, order tests, prescribe medications, help you manage chronic conditions and coordinate specialty care when needed. We are here to help provide the tools and support our community needs to live healthier lives. In other words, primary care is your first line of defense against day-to-day illnesses and chronic conditions.
We are accepting new patients from 18 years old and up. If seniors are not eligible for Medicare, they are welcome to use services at the clinic. If seniors have Medicare without Part D, the clinic can help them access expensive brand name medications.
And if your primary care provider determines you need an evaluation by a specialist, like a cardiologist, neurologist or other specialists, we’ll refer you to one.
Pharmacy
The dispensing pharmacy provides medications for on-going health conditions – diabetes, heart disease, and mental health conditions. No controlled medications (narcotics) are dispensed.
Chronic Disease Management
A Physician and a Nurse Practitioner are available to provide primary (non-emergency) medical care and treatment for chronic conditions such as diabetes & high blood pressure.
Medication Assistance
Our Prescription Assistance Program can help you get the long-term brand medications you need but cannot afford, through applications to the programs of the major pharmaceutical companies. All programs are based on income, and the requirements are different for each company.
The Way It Works
We will enter your basic information as well as financial information into The Pharmacy Connection. Next, we will enter each prescription to determine both the availability of the medication and your eligibility with the drug company. However, not all medications are available through this program. If you are eligible, a signature from you and your physician is required. The application is then submitted to the drug company. The process takes about six weeks before the medication will be delivered back to the Community Care Clinic. After the initial process, refills should arrive more quickly. Finally, we will call you when the medication is available for pick-up.
